Eligible applicants must demonstrate existing infrastructure and current care and services in at least two of the five primary drivers of comprehensive dementia care and services and identify new opportunities and additional services to enhance and strengthen existing dementia care approaches in a clinical setting. The five primary drivers of comprehensive dementia care include 1) Awareness and Recognition, 2) Accurate and timely diagnosis, 3) Interdisciplinary assessment, 4) Management and referral, and 5) Caregiver support.
