The project which has a 64 month duration has a partnership of 6 drawing from Northern Ireland, Border Region of Ireland and Scotland. The project will be implemented across the eligible region of Northern Ireland, Border Region of Ireland and Western Scotland. Co-H Sync is short for “synchronise” and accordingly this project aims to synchronise the efforts of the community, voluntary and statutory sectors in order to impact positively on the health and well-being of individuals and communities, empowering and supporting local communities to manage their own health needs. This will be done through the implementation of initiatives within 8 hub areas which focus on early intervention and prevention of ill health which will assist people to improve their health behaviours and habits through Personal Health Plans. In addition the project seeks to improve health literacy, provide community forums in each of the 8 hubs including 2 cross-border forums; develop a cross-border network of local health trainers and develop social media, apps, e-health tools and information sources. The objectives of the project are to: Improve the health literacy of the target populations; Assist people to improve their lives by enabling them to find ways to sustainably better their health and well-being within a supportive community framework; Break down the barriers between the statutory and community sectors by creating locally based health and well-being Community Hubs; Develop the local health and well-being Community Hub to become the touchstone for the community’s health and well-being; and, Explore and utilise novel approaches to health behaviour change for deprived populations, thereby reducing health inequalities.
