Description
Description | PurposeThe intent of this funding opportunity is to award entities to focus on specific targeted partnerships and innovative strategies to reduce falls and/or falls risk among the majority being older adults (60 years and older) and adults with disabilities, with a focus on those with greatest social and/or greatest economic needs (i.e., target population for this Funding) through the implementation of evidence-based falls prevention programs with an innovative person-centered approach. This funding opportunity has two goals:Goal 1: Develop an innovative fall prevention method that includes targeted partnerships and outreach to recruit the target population into small group in-person evidence-based falls prevention programs.Goal 2: Design an evaluation of your intervention to assess the impact of the effort on reducing falls and/or the risk of falls, and develop and disseminate 508-compliant program results, resources, lessons learned, and process documents to inform practice, service delivery, program development and/or policymaking for other organizations to replicate this approach, to include a comprehensive list of partnerships developed. The Administration on Aging (AoA), part of the Administration for Community Living (ACL), plans to award 2 cooperative agreements with a three-year project period, subject to availability of funds. |
