A significant proportion of the elderly population in cross-border rural areas suffer from chronic diseases that can lead to functional decline and a need for coordinated care. The HIS2R project, focused on the Ardennes region, aims to improve the health of chronic patients, the comfort and collaboration of providers, and reduce costs for patients and social security. It also aims to optimise cross-border coordination of care in rural areas, with the support of new technologies (connected objects and efficient and secure transmission of information). Patients with chronic heart failure or chronic obstructive bronchopathy are equipped with a device to collect and transmit health information and alerts in a secure manner to caregivers. A system of televigilance and case managers makes it possible to react to alerts in real time. Patient data can be viewed on both sides of the border.
