CDC Foundation (“Contractor”) shall provide the following specific services: a. RHTP Project Management and Advisory Services - Under the direction of RHTP leadership at DSS, the Contractor shall provide the following Services: i. Implement DSS-led RHTP projects including guiding subcontractors, engaging with stakeholders, and ensuring DSS contractors’ compliance with the RHTP Notice of Award including federal requirements, goals, and milestones ii. Oversee and coordinate the work of the external evaluation contractor for overall RHTP evaluation. iii. Coordinate with other state agencies implementing RHTP projects in monitoring all RHTP projects to ensure compliance with federal requirements, goals, and milestones. iv. Provide program monitoring and evaluation support for all DSS-led RHTP projects, including data collection, analysis, and reporting efforts to ensure projects are evidence driven and aligned with RHTP goals. v. Develop and implement a comprehensive outreach and communications strategy for RHTP across all agencies and projects, including development and distribution of weekly updates to recipients from the DSS-developed RHTP list serve, coordinating CMS approval of all RHTP-related presentations and communications, and developing and maintaining a calendar for social media promotions. vi. Provide clinical and technical expertise to support RHTP project objectives to strengthen health care delivery, including collaboration with project stakeholders to identify gaps and enhance care coordination.
