Description | Department of Veterans Affairs, Network Contracting Office 10 located at 260 E University Avenue, Cincinnati, OH 45219-2356, in support of Veterans Integrated Service Network (VISN) 10, Chillicothe VA Medical Center (VAMC) Pathology & Laboratory Medicine Service (P&LMS), located at 17273 State Route 104, Chillicothe, OH 45601-9718 awarded a firm fixed price Federal Supply Schedule (FSS) Blanket Purchase Agreement (BPA) for annual Hemoglobin A1c Cost Per Reportable Result (CPRR) testing to Tosoh Bioscience, Inc. under FSS Contract V797D-40130. Tosoh Bioscience, Inc Large Business 360 Gantz Road Grove City, Ohio 43123-1895 The acquisition includes the supply of test kits, contractor owned equipment, supplies, and customer support to perform in-house testing services for veteran patients. The period of performance (POP) will be a base year with four 1-Year ordering options. The annual cost is $27,000.00. The total cost including all option years is $135,000.00. |
