The purpose of this memo is to make you aware that the Mukwonago Area School District ( MASD ) is accepting proposals for Before & After School Daycare Services for the two upcoming school years . f your organization would like to submit a proposal , we would ask that it contain the following information :• Company / program background and experience • A summary of intended activities and program goals • Staffing levels and personnel information , including ratio of employees to children • Parent fee / tuition listing , including intermittent attendance • Wisconsin Department of Children & Families licensure , non - compliance reports , and YoungStar ratings ( if applicable ) • Any other credentials , references , survey results , or recommendations / testimonials • A list of MASD elementary schools you are interested in providing service ; for each school , please include : • the number and ages of the students to be served • the arrival & departure schedule • an assurance that the day care provider will be responsible for actual incremental costs , will be liable for any damages incurred , and will not provide religious instruction or permit religious practices to be conducted during the day care program
