Blueprint for Health Primary Care Technical Assistance

Through this Request for Proposal ( RFP ) the Department of Vermont Health Access ( hereinafter the “ State ”) is seeking to establish contracts with one or more companies that can provide primary care practices the expertise , tools , and training to improve financial sustainability , navigate complex payment systems , build capacity to improve overall operations , and recognize opportunities for cost efficiency and containment . The selected vendor will provide financial technical assistance for up to 24 primary care practices , first prioritizing the primary care practices that engaged in previous discovery conversations with an AHS contractor to provide primary care technical assistance in 2025 . The vendor will meet with each practice to engage relevant team members from the practice , determine specific financial needs and goals through an assessment process , develop an action plan and timeline for selected objectives and strategies , and draft / review the plan with the practice . The vendor will provide the board / staff education structured on strengthening organizational financial accountability through financial monitoring , revenue recovery , risk - based auditing , cost containment , and strategic planning and specific to the assessment findings , recommendations , and implementation requirements . The vendor will prepare the practice / organization to be able to implement the plan and self - monitor outcomes . The vendor will conduct a post - implementation evaluation with the practice 6 months after implementation . The vendor will support practices in developing sustainability plans in the following areas of financial operations , including , but not exclusive to : • Revenue cycle management : Assessing and improving all aspects of the revenue cycle , including billing and coding , claims management , value - based program participation , reducing claims denials , and optimizing collection rates . • Cost control : highlighting opportunities for cost efficiencies , with an emphasis on exploring shared purchasing opportunities for example , in domains such as human resources ( information systems ), information technology ( platforms and analytic support ), insurance / benefits , billing and coding , supply chain , utilities ( fuel , laundry , waste disposal ), marketing , clinical education , and credentialling through existing or new collaboratives ). • Budgeting and forecasting : Developing multi - year budgets and financial forecasts to support strategic decisions and planning . o Evaluating and assisting with strategic planning for services that are currently not currently covered through insurance . o Evaluating staffing mix and revenue / compensation opportunities .
