Description
Description | This Contract is funded by federal funding from the CMS Rural Health Transformation Program (“RHTP”). State and federal laws, regulations and rules apply to this Contract. Section 71401 of Public Law 119-21, codified at 42 U.S.C. 1397ee(h), established the RHTP to help empower States to strengthen rural communities by improving healthcare access, quality, and outcomes by transforming the healthcare delivery ecosystem. The State of Connecticut Department of Social Services is the recipient of funds under an award of federal grant funds from the U.S. Department of Health and Human Services (“HHS”), Centers for Medicare and Medicaid Services (“CMS”), for the purpose of implementing the RTHP in association with multiple state agencies. The Contractor’s contributions shall support the Health Information Technology Officer (“HITO”) and OPM in ensuring that Connecticut’s HIT work under RHTP is aligned, coordinated, interoperable, and actionable. Including but not limited to: 1. Lack of real-time visibility into hospital bed status; 2. Emergency department boarding and transfer delays; 3. Fragmented capacity reporting processes; 4. Limited preparedness during surge, weather, or mass-casualty events; and 5. Need for a validated, evidence-based design to guide future implementation. |
