Contractor shall provide to DSS “evidence” of its ability to either directly, or indirectly, through a DCO, provide the following 9 mandatory CCBHC services: i. Community-based mental health care for veterans ii. Crisis Services iii. Outpatient mental health and substance use services iv. Outpatient primary care screening and monitoring v. Peer, family support, and counselor services vi. Person- and family-centered treatment planning vii. Psychiatric rehabilitation services viii. Screening, diagnosis and risk assessment ix. Targeted case management b. Evidence of the Contractor’s ability to provide directly the mandatory CCBHC services shall consist of proof of a positive accreditation status decision by NCQA. Evidence of the Contractor’s ability to provide indirectly mandatory CCBHC services shall consist of a copy of a subcontract between the Contractor and a DCO under which the DCO agrees to provide delegated mandatory CCBHC services on behalf of the Contractor.
