Description | 3 . 1 Provide chronic disease self - management programming statewide or within a specific region of the state of South Dakota . This would include but is not limited to : 3 . 1 . 1 Maintain a license for an evidence - based Chronic Disease Self - Management ( CDSM ) Program throughout the project period . 3 . 1 . 2 Offer CDSM lay leader trainings . 3 . 1 . 3 Plan for CDSM workshops in rural South Dakota communities and offer technical assistance . These may be held in - person or virtually . 3 . 1 . 4 Develop and implement a marketing plan to engage local participation in CDSM workshops , especially in rural communities . 3 . 2 Provide a monthly report , including progress related to contract deliverables and scope of work to the SD DOH . 3 . 3 Participate in monthly programmatic meetings as directed by SD DOH . |
