Description

Dakota County Community Services is seeking to contract with one or more established community agencies with 5 + years of case management experience with at least 1 other metro county to provide case management for individuals with disabilities and older adults who receive funding through one of the following home and community - based services programs and / or county funded case management : Community Access for Disability Inclusion ( CADI ), Community Alternative Care ( CAC ), Brain Injury ( BI ), Developmental Disabilities ( DD ), Non - Waiver County Funded Rule 185 , Elderly Waiver ( EW ) or the Alternative Care ( AC ) program . Case management services include but are not limited to , informing the individuals or the individual ’ s legal representative of service options , identification of potential service providers , development of a service plan based on the individual ’ s assessed needs and preferences , accessing and coordinating services , and evaluation and monitoring of services identified in the individual ’ s plan . Case management for these individuals will require travel throughout the Metro area .
