Data Support , CQI & Evaluation Services for the Reentry Services Program

The County of Camden is requesting the following services to work with the Camden County Correctional Facility ( CCCF ) and selected vendor to support continuous quality improvement and conduct evaluation of Camden County Department of Corrections COSSUP reentry program . The County is working to procure an agency to provide the reentry program services . The CCCF was awarded a COSSUP grant from the Department of Justice , Bureau of Justice Assistance to operate a reentry program for individuals with substance use disorders returning home following a period of incarceration . The scope of services is to provide data support , continuous quality improvement , and evaluation for the entire grant period for the reentry program services . Responsibilities shall include : 1 . NARRATIVE – Key Components and Elements a . Work , as a member , with the Reentry Data Evaluation Team to reach consensus on a comprehensive set of measures to be tracked for all components of the grant program . This must include both program and process measures and outcomes of all program participants . Some identified expected outcomes include but not limited to :  Increase in % of jail population screened and assessed for substance use .  Increase in % of individuals receiving SU treatment in CCCF .  Increase in # of individuals who receive transitional or recovery housing .  Increase in # of individuals connected to peer support teams .  # of peers hired and trained  Increase in # of individuals connected with children and / or families .  Increase in # of individuals connected to community - based treatment upon release and continue treatment .  Increase in # of individuals that find and maintain employment .
