The West Virginia Department of Education ( WVDE ) is soliciting a request for quotes ( RFQ ) on behalf of the West Virginia Schools for Diversion and Transition ( WVSDT ) and West Virginia Schools for the Deaf and Blind ( WVSDB ) as indicated below . 4 . 1 . 1 . The Vendor must provide a single vendor , turn - key , Layer 2 solution . The project must include termination of each circuit at equipment owned and maintained by the vendor at each termination location ( i . e . school site ). Quoted pricing must include costs associated with providing the service , including but not limited to special construction charges , installation , any applicable upfront , non - recurring costs associated with the installation of new fiber to or between eligible entities , and monthly recurring services of high bandwidth data transmission services that will transport full quoted bandwidth for the WVDE schools ’ use only , without Internet access that : 4 . 1 . 1 . 1 . The vendor must be an eligible provider under the FCC / SLD rules and be willing to file an E - rate Form 474 , Service Provider Invoice ( SPI ) on behalf of the WVDE and / or each state school Local Education Agency LEA . Documentation of this should be included with the bid response , along with the vendor ’ s Service Provider Identification Number ( SPIN ) which must be provided for application purposes . 4 . 1 . 1 . 2 .
