The Consultant shall perform ( but not limited to ) the following tasks : 3 . 1 Program Planning and Design • Program Guidelines : Refine comprehensive Grant Program Guidelines , including eligibility criteria , funding priorities , allowable costs , scoring rubrics , budget template , and application timeline . • Performance Indicators : Monitor and report on measurable metrics as defined in the Rural Health Transformation application for all grant recipients ( e . g ., EHR adoption rate , interoperability standards met , specific health outcome improvements ) and for the Program ’ s administrative success . • Documentation and Templates : Update informational website for all necessary templates , grant guidance , application forms , reporting templates , and FAQs , etc . • Comprehensive Statewide EHR Adoption Assessment : Within 120 days develop and complete a comprehensive assessment of EHR adoption by health care provider types . This assessment shall include all health care stakeholders in SD . 3 . 2 Grant Solicitation and Applicant Support • Program Launch and Outreach : Manage marketing , including developing communication plans , hosting informational webinars , and managing an Applicant Q & A process and participating in Community Recruitment and Regional Conferences and events statewide . • Technical Assistance : Provide responsive technical assistance to potential applicants regarding eligibility , application requirements , and other technical requests . 3 . 3 Application Review and Award • Review Committee Management : Recruit , train , and manage a Grant Review Committee comprised of subject matter experts . • Scoring and Evaluation : Oversee the impartial application evaluation process , ensuring all scoring aligns with the approved rubric and documentation standards . • Recommendation and Award : Consolidate review scores , provide funding recommendations to SDHL leadership , and manage the formal grant award notification process ...
