he State of Hawaii , Department of Health Environmental Management Division Clean Air Branch is seeking aqualified bidder to provide a quote on a NEW 10 ' x 20 ' custom modular elevated aluminum platform with access stairway and OSHA - compliant railings and toe boards ( TYP ), FOB jobsite . Said quote shall also include furnishing two ( 2 ) each modular trailer access stairs and platforms w / railings . The materials are to be delivered to : the State of Hawaii Department of Health , Safe Drinking Water Branch ( SDWB ) laydown yard at 2385 Waimano Home Road , Pearl City , Hawaii 96782 . Qualified bidders must have prior experience providing and delivering mobile modular units . Bidders must briefly describe their prior successful completion of similar project ( s ). Overview of Services Requested : · Furnish and delivery of one modular aluminum 10 ' x 20 ' x 5 ' finished floor height elevated platform with one access stairway and OSHA - compliant railings and toeboards ( TYP ), complete with all necessary materials and proprietary tooling to fully assemble and install said platform . · Furnish and deliver two ( 2 ) modular aluminum trailer access stair and platform assemblies for a finish floor height of 36 inches ( TYP ). Both assemblies shall be complete with OSHA - compliant railings and toeboards , and shall come with all necessary materials and proprietary tooling to fully assemble and install said assemblies . cope of Work : Overview - All materials provided must be new / in unused condition . See attached plan view photo # 1 / sheet 3 of proposed elevated platform design . See attached photos # 2 & # 3 / sheet 3 of the trailer access ladder assemblies . · Furnish and deliver one modular aluminum 10 ' x 20 ' x 5 ' finished floor height elevated platform with one access stairway and OSHA - compliant railings and toeboards ( TYP ), complete with all necessary materials and proprietary tooling to fully assemble and install said platform . The contractor shall be responsible for ensuring the condition of said materials until acceptance at time of delivery onsite . Furnish and deliver the following materials for use inthe assembly of the modular platform , FOB jobsite :·· 18 each , 16 " x16 " x6 " TILECO concretefoundation blocks or equal . ·· 24 each , HILTI 3 / 8 " x4 - 1 / 2 " Kwik - Bolt zinc - plated steel long - thread concrete wedge anchor assemblies with washer and hex nuts ( or equal ). CONTRACTOR SHALL CONFIRMTHE DIAMETER SIZE OF THE ANCHORS TO FIT THE PEDESTAL ANCHOR HOLES OF THE PLATFORM · Furnish and deliver two ( 2 ) modular aluminum trailer access stair and platform assemblies for a finish floor height of 36 inches ( TYP ). Both assemblies shall be complete with OSHA - compliant railings and toeboards , and shall come with all necessary materials and proprietary tooling to fully assemble and install said assemblies . The contractor shall be responsible for ensuring the condition of said materials until acceptance at time of delivery onsite . · All components and systems included must comply with all applicable federal , state and local codes and requirements including ( but limited to ): OSHA , UBC , NEC , NFPA and Hawaii State Building Code ( 4 / 2021 ).
