Description
Description | Goal 1 : Support Successful Community Reintegration and Reduce Recidivism The Pathways Forward Mentoring Program seeks to promote successful reintegration for youth returning from placement by providing consistent , relationshipbased mentoring and structured support during the critical transition period . Beginning prior to release and continuing through community reentry , mentors will work closely with youth to build trust , reinforce accountability , and connect participants to positive community resources . Through sustained engagement , pro - social activities , and coordinated support , the program aims to address key risk factors associated with reoffending while strengthening protective factors that support long - term stability . Progress will be monitored through participation data and ongoing tracking of recidivism indicators during program involvement . Goal 2 : Increase Workforce Readiness and Pathways to Economic Stability The program will enhance workforce readiness by equipping youth with the skills , knowledge , and supports necessary to pursue employment or educational opportunities . Mentors will provide individualized guidance focused on career exploration , job - readiness preparation , and overcoming common employment barriers faced by justice - involved youth . Participants will receive assistance accessing workforce training , internships , or educational pathways aligned with their goals and abilities . By promoting early engagement in productive activities and strengthening employment - related skills , the program aims to increase economic stability and support positive long - term outcomes . Goal 3 : Promote Youth Stability , Social - Emotional Development , and Positive Decision - Making Pathways Forward emphasizes holistic youth development by fostering social - emotional growth , positive decision - making , and personal responsibility . Through regular mentoring interactions , youth will engage in goal setting , problem - solving , and skillbuilding activities that strengthen self - awareness and interpersonal skills . Structured pro - social activities will provide opportunities for healthy peer engagement and community connection following release . Additionally , limited stabilization supports , including emergency assistance , will help address immediate barriers that could disrupt progress . Together , these strategies aim to support youth in building resilience , maintaining stability , and successfully navigating their transition back into the community . |
