The contractor shall provide long - term care rebalancing opportunity services in the region ( s ) awarded as stated on the contractor ’ s Notice of Award . The contractor shall be available by phone during regular business hours , 8 : 00 A . M . through 5 : 00 P . M . CST , and after hours as needed by the state agency . The contractor shall respond to all emails sent by the state agency within a timely manner , not to exceed three ( 3 ) business days . The contractor shall meet in - person or by alternative methods with the state agency as requested by the state agency . If the state agency requests an in - person meeting to include all awarded contractors , the state agency will choose a central location to meet . The contractor shall provide at a minimum three ( 3 ) professional personnel to provide the long - term care rebalancing opportunity services herein . The contractor shall monitor the performance of all professional personnel and the MFP Supervisor to assure compliance with the terms of the contract . The contractor shall implement a system of targeted outreach and education to skilled nursing facilities ( SNF ) within the contractor ’ s awarded region and track such outreach contacts and outcomes and have feedback available upon request by the state agency .
