Two One - Page - Descriptions . 2 . Credentialing Plan and Learning Plan . 3 . Letters of recommendation from both sponsoring Mentors . Time Required : 1 . 5 hours Second Mentor ( remote ); Step 2 : Mentor Candidate Orientation : The Second Mentor meets with the Lead Mentor and the Mentor Candidate to review : • The Mentor ( s ) and Mentor Candidate Donut to clarify role responsibilities . • Feedback forms for the PCT Trainees and the Mentor candidate . • Establish communication methods and frequency . Time Required : 1 . 5 hours Second Mentor ( remote ) |
