Description | The State of Delaware Department of Health and Social Services , Division of Public Health is requesting proposals to develop and conduct appropriate statewide activities that will advocate and encourage improved health outcomes . Successful applications will propose evidence , practice , or research - based strategies that support expanding inclusivity and improve health equity . These strategies can include : • Implementing recommendations in The Impact of Diabetes in Delaware , 2025 biannual report , The Burden of Chronic Disease in Delaware 2024 , and the Delaware Cancer Consortium ( DCC ) state cancer goals and objectives found in the Five - Year Cancer Plan 2022 - 2026 . • Facilitating the vision to make Delaware the healthiest state in the nation as outlined in the Delaware State Health Improvement Plan ( SHIP ) 2025 - 2028 . The RFP awardee will be responsible for demonstrating a comprehensive methodology improving the health of Delawareans through prevention and management of cardiovascular health ( CVH ). |
