Description | NAVIGATE is a CSC treatment model developed by the RAISE Early Treatment Program research team . This fidelity - oriented Coordinated Specialty Care model focuses on a collaborative , multidisciplinary approach to treatment . The target population is individuals aged 15 - 35 who have experienced FEP within the previous eighteen ( 18 ) months . Under the NAVIGATE CSC model of care , service components may include : • Family Education • Prescriber • Individual Resiliency Training • Supported Employment and Education services • Peer Support & Family Support • Care Management / Nursing . Under the NAVIGATE model , contracted teams are trained in the NAVIGATE model and then implement the program within their geographical area . The intent of this RFP is to establish a subaward to an organization which will expand services in one new geographical region . |
