NBS Form and Printing Specifications Contractor shall supply NBS lab collection forms and serve as the designated printer entity that operates through a Federal Food and drug Administration ( FDA ) approved facility that meets the International Organization for Standardization ( ISO ) 9001 - 2008 certification ’ s standards and ensures its subcontracted printer entity ( if applicable ) is in compliance with the aforementioned standards , as specified in this eRFQ and the contract agreement . Contractor shall supply NBS forms that meet the following construction , language , formatting , and size specifications : a ) Construction : Six ( 6 ) part screening forms construction and language ( the specific multi - color printed language and form field requirements to be printed on the forms are provided in this eRFQ ) requirements : a . Part 1 – Cover of form , 15lb carbonless bond b . Part 2 – Lab Data Entry Copy , 20lb or 21lb coated front and back , white c . Part 3 – Submitter ’ s Copy , 14lb carbonless bond d . Part 4 – Lab Copy , 105lb Manila Carbonless . e . Part 5 – Filter paper attached to Part 4 , filter paper approved by the FDA for blood spot collection and obtained from an FDA cleared source . Each lot of filter paper must be certified by the Centers for Disease Control and Prevention ( CDC ) as meeting the required performance standards set forth in CLSI - LA4 - A5 prior to putting the lot into use . The filter paper must be Ahlstrom 226 or Whatman 903 . f . Part 6 – Back of Form , 32lb buff , ledger .
