Description | OHS is seeking to expand and sustain long - term , affordable supportive housing for individuals and families with services to help them end homelessness and maintain their housing . The following are the goals OHS expects to achieve through the work of the selected Applicants : • Provide safe , affordable housing to end homelessness for individuals and families . • The target rate for the number of participants who retained Supportive Housing / Exits to Supportive Housing will be 93 % or more ; meaning that during the contract period , at least 93 % of participants will either remain in the program or will exit to other permanent housing . • Ensure participants who are housed through this RFP do not return to homelessness . |
