Objectives of this service include the following : 1 . The total number of disrupted adoptions will decrease . 2 . There will be a single point of contact for information , support , and direction . 5 3 . Information and resources will be provided to adopted children and families seeking community resources . 4 . Consultation and intervention will be provided to adopted children and families in crisis . 5 . Adopted children and families will be connected to community resources . 6 . Advocacy and system navigation will be provided for adopted children and families experiencing a crisis . 7 . Adoptive placements will be stabilized by connecting families to supportive services in the community during the period of supervision to prevent disruptions or to prevent dissolutions after finalization . 8 . Adopted children and families will remain intact by preventing the child from entering or re - entering the foster care system . 9 . Building strong collaboration with child welfare partners to strengthen the service continuum for families and children
