SCOPE OF WORK ( SOW ) COC is seeking proposals from qualified , licensed and insured entities to provide the following detailed services listed herein : 2 . 1 . 1 Coordinate the recruitment , screening , and placement of such direct hires and temporary employees in accordance with the job descriptions provided by COC ( PLEASE NOTE : COC may choose to refer preferred candidates to the firm for any such retention ; and there is an option herein to provide direct placement of certain employees ). 2 . 1 . 2 During the term of the contract , coordinate , maintain and / or administer : The administration of and payments to such temporary employees ( COC will set the range / rate of compensation for each such temporary employee ). 2 . 1 . 3 COC will set the range / rate of compensation for each direct hire . 2 . 1 . 4 The withholding and payment to the appropriate governmental agencies and reporting of all taxes and issuance to the employee of the W - 2 forms at the end of each year , each as required by law . 2 . 1 . 5 For each such temporary employee , insurance coverage as further detailed herein insurance claims pertaining to the noted insurance coverage . 2 . 1 . 6 Coordinate the screening of such temporary and / or direct hire employees to include criminal background check ; education verification ; hair analysis drug screening ; and , when directed by COC for specific positions that may require such , pre - employment physical . If the person in the position is required to drive COC vehicles , a Department of Motor Vehicle ( OMV ) check will be required . Also , according to the State of Connecticut Health Department , Food Service is required . 2 . 1 . 7 As compensation for providing the services pertaining to temporary and / or direct hire employees , the successful proposer ( s ) will charge COC a set percentage of each such " rate of pay / salary " set by COC , Temporary employee invoices must be submitted weekly . Direct hire invoices must be submitted 1 week thereafter the start date .
