SCOPE OF WORK SECTION General Requirements : The contractor shall provide Respiratory Therapy Services for the consumers of the South County Habilitation Center and the Bellefontaine Habilitation Center in accordance with the provisions and requirements stated herein and to the sole satisfaction of the state agency . If requested by other state agency facilities within the St . Louis region and if the contractor is available the contractor shall provide a respiratory therapy services at other state agency facilities within the St . Louis region . Such facilities may consist of the other St . Louis Developmental Disabilities Treatment Center ( SLDDTC ) locations . The contractor shall provide respiratory therapy services on an as needed , if needed basis and to the sole satisfaction of the state agency . Contractor ’ s Obligation : Unless otherwise specified herein , the contractor shall furnish all material , labor , facilities , equipment , and supplies necessary to perform the services required herein . Contractor Qualifications : The contractor should have a minimum of three years of experience providing respiratory therapists to residential facilities . Respiratory Therapist Qualifications : The contractor ’ s respiratory therapists must meet the following minimum qualifications : be a Certified Respiratory Therapist with the National Board of Respiratory Care ; have Medicare / Medicaid credentials ; and have two years of experience as a respiratory therapist ; and have knowledge and two year of experience performing all duties described in the respiratory therapist performance requirements herein . Prior to providing services and as requested by the state agency , the contractor must provide current copies of licensure and credentials to the state agency for the Professional Licensure Notebook maintained at the state agency ’ s personnel office . At the request of the state agency , the contractor shall provide references , resumes , test scores , or any other applicable requested information for the contractor ’ s respiratory therapists . The state agency reserves the right to conduct interviews with selected individuals after the review of such information . The state agency reserves the right to reject any or all individuals selected by the contractor for the provision of services under the contract . The state agency shall provide justification for each rejection . If requested by the state agency , the contractor shall replace any respiratory therapists with whom the state agency is not satisfied
