A . Intent To establish a multi - term contract for the purchase and delivery of Rubber Goods Cleaning , Testing and Replacement on a as need basis . B . Minimum Requirements 1 . The following bid is made for furnishing materials or services for the City of Farmington , San Juan County , New Mexico . 2 . The Bidder declares that the amount and nature of the materials to be furnished is understood and that the nature of this bid is in strict accordance with the conditions set forth and is a part of this bid , and that there will at no time be a misunderstanding as to the intent of the specifications or conditions to be overcome or pleaded after the bids are opened . 3 . All items quoted must meet or exceed City ’ s written specifications , or in their absence , all applicable ASTM specifications . 4 . All items quoted must include Manufacturer ’ s name , product number or description , and delivery lead time . C . Specific Requirements 1 . All items , and testing of items , must meet or exceed current OSHA standards and requirements . 2 . Quotes shall include all testing standards / methods used for protective equipment listed . 3 . Records of tests performed shall be sent with all returned product . 4 . Records will indicate a pass or fail status and all failed product will be marked reject or failed with failure spot clearly identified . 5 . Failed items shall be returned to the City for inspection . Any item that fails testing or inspection shall be returned “ as is ” and marked as stated in C . 4 6 . Any replacement of protective equipment ( including gloves ), shall be the same manufacturer and part / item # as failed protective equipment . 7 . Replacement of failed rubber glove or gloves will be included in return shipping . 8 . Replacement glove or gloves will be numbered / identified identical to the failed glove ( s ) it is replacing . 9 . Rubber gloves will be individually paired with their like number and bagged . 10 . Any rubber insulating equipment , ( excluding gloves ), tested and proven to have failed , will have a quote for its replacement sent to the City contact prior to new insulating equipment being ordered for replacement . 11 . Bidder shall arrange for the pick - up of items to be cleaned and tested . Pricing shall include the cost of pick - up and / or freight to and from the testing lab .
