The Tennessee Department of Education (“ State ”) will award one ( 1 ) grant contract to carry out the full scope of work outlined below and further detailed in the attached pro forma contract . The attached pro forma contract constitutes the substantive terms and conditions , and the awarded grantee will be required to sign and execute the contract as part of the agreement . The purpose of this grant contract is to ensure compliance with federal and state requirements related to transition services and postschool outcomes for students with disabilities in Tennessee . Specifically , the awarded grantee shall support the State in fulfilling data collection , analysis , compliance monitoring , and reporting requirements for Annual Performance Report ( APR ) Indicator 13 ( Secondary Transition ) and Indicator 14 ( Postschool Outcomes ). Under this grant contract , the awarded grantee shall : • Review student IEP transition plans , as required under Indicator 13 , to determine compliance with applicable federal and state laws , including the Individuals with Disabilities Education Act ( IDEA ) ( 20 U . S . C . § 1400 et seq .) and SBE Rule 0520 - 01 - 09 - . 12 ( 2 )( b )-( c ); • Report any findings of noncompliance with Indicator 13 requirements to the State ; • Conduct systemic reviews to assess the overall effectiveness and compliance of district - level transition planning for Indicator 13 , separate from the annual studentlevel reviews ; • Distribute the Indicator 14 postschool outcomes survey to designated Local Education Agencies ( LEAs ) annually , including compiling a list of survey participants and any other necessary resources to support survey completion ; • Collect and analyze Indicator 14 survey responses ; • Report the results of Indicator 14 data collection to the State in accordance with timelines and data quality standards ; • Develop and submit annual statewide Indicator 13 and Indicator 14 reports to the State . The state will then review and verify the reports and and distribute the individual LEA - level Indicator 13 and 14 reports to each respective LEA through ePlan .
