SCOPE OF WORK 3 . 1 Work closely with the South Dakota Department of Health Heart Disease and Stroke Program to facilitate the Cardiovascular Collaborative , including providing support and technical assistance to the Cardiovascular Collaborative Leadership Team and strategic plan Goal Area Workgroups as they develop , implement , and evaluate interventions addressing the statewide cardiovascular strategic plan . 3 . 2 Facilitate the development of a new Cardiovascular Collaborative Strategic Plan to be implemented in 2027 . 3 . 3 Collaborate with the Heart Disease & Stroke Program Coordinator to plan and host in - person and virtual meetings for the Cardiovascular Collaborative , providing training , education , and reviewing successes and progress related to the strategic plan . 3 . 4 Create and distribute up to four newsletters to Cardiovascular Collaborative members to inform them of relevant news and heart disease and stroke happenings in South Dakota . 3 . 5 Provide strategic guidance to the Cardiac Ready Communities workgroup . 3 . 6 Participate in monthly meetings with the Heart Disease and Stroke Program Coordinator . 3 . 7 Submit monthly progress reports by the 15th of each month using the reporting template provided by the South Dakota Department of Health . Reports should summarize activities , outreach , outcomes , and performance measures as specified in the contract .
