Description | De gemeente Utrecht wil één (1) overeenkomst afsluiten voor de Wmo ondersteuning in de wijk met als doel dat inwoners, ongeacht hun ondersteuningsvraag, verantwoord zelfstandig kunnen gaan of blijven wonen. De ondersteuning sluit aan op het gewone dagelijks leven. Daarbij is aandacht voor het versterken en onderhouden van een stevig sociaal netwerk. Deze opdracht betreft delen van de aanvullende zorg Wmo, de zorg die aanvullend is op het aanbod vanuit de sociale basis en de basiszorg vanuit de buurtteams en het bieden van specialistische en laagdrempelige dagactiviteiten als algemene voorziening. Zie paragraaf 2.1.4 en 2.1.5. van het inkoopdocument. |
